This document is complied of the recommendations from the 3rd edition of the IAFF/IAFC Wellness Fitness Initiative (WFI).  The form is a tool to use with your doctor to ensure your physical meets the intent of the WFI as it recognizes the higher risks of a firefighters profession.The form includes common codes associated with the procedure to ensure your physician performs recommended exam.

Please note that some of the procedures are outside what your insurance will normally cover so you may incur some expenses for those procedures.  It is recommended you have a discussion with your physician prior to conducting the tests to get an estimate of costs.
